The information collected will only be used for Telecare Crisis & Caring Line records and will not be released to outside organizations in compliance with the privacy law of BC (January 2004)

Important Information Before You Submit This Application

By digitally submitting this application form, you are indicating that you have read and agree with the Ministry Agreement, Faith Policy, and Statement regarding Confidentiality (see links below).

By filling out and digitally submitting this application you are also indicating that you:

• intend to pay an application fee ($100) See below.
• intend to submit a criminal record check (we will provide you with a sample letter)
• intend to provide a reference from a credible source
• intend to volunteer 100 hours within the coming year (approximately one shift per week)
• See the Telecare Ministry agreement below for more commitments

We will send you information on how to do all the three items above once we receive your application form.
